Your Application will require a Provincial Health card and one of the following:

• Valid Canadian passport;

• A birth certificate issued by a Canadian provincial, territorial, or federal government agency;

• A valid identification card, visa or permit (work, study, resident, etc.), with picture, delivered by a Canadian federal government agency which legally authorizes the holder to reside in Canada for an extended period;

• A valid identification card, with or without a picture, issued by a Canadian federal, provincial, or territorial government agency
