The button will take you to the online registration form through Canada.ca, where you can also come to the unit on any Tuesday and one of the staff will be happy to help you fill out a physical copy.

You must bring 2 pieces of ID to finish the application: a provincial/Territorial health card (OHIP) ​and one of the following:

• Valid Canadian passport;

• A birth certificate issued by a Canadian provincial, territorial, or federal government agency;

• A valid identification card, visa or permit (work, study, resident, etc.), with picture, delivered by a Canadian federal government agency which legally authorizes the holder to reside in Canada for an extended period of time;

• A valid identification card, with or without a picture, issued by a Canadian federal, provincial, or territorial government agency
